1. Full membership shall be open to all DMV residents who commit themselves to abide by
the Goshen Benevolence Association (GBA) by-laws, its regulations and its mission
statement.
2. Prospective members shall become active members upon fulfilling application, admissions
and financial requirements.
3. To remain active, a member MUST be current financially in the books of record without any
outstanding balances in their coverage.
4. For records and claim purposes only, each member shall be issued with an identification
Number (ID Number)
Membership Application and Admission
- Each applicant MUST be at least 18 years to apply.
- Each applicant/s MUST submit filled out Goshen Benevolence Association (GBA) application form physically or electronically on the Goshen website. Incomplete application forms will delay the approval process
- Each applicant MUST list ALL names of close relatives to be covered. Close relatives here refer to husband, wife, son, daughter, mother, father, brother and sister ONLY. NO EXCEPTIONS.
- Each member MUST sign the application form and waiver form before submission as self-evidence to abide by the terms and conditions as set fully by GBA by- laws and guidelines. If need be, staff will complete a notary attestation.
- All members MUST pay $ 100.00 Non-refundable fees for registration during the open enrolment in the month of (August), and $100 contribution following the demise of a member/family.
NB: Upon reaching 200 members, contributions will be reduced to $50 per member and further reevaluated year and as needed to low contribution based on membership numbers.
- No annual feed All members unless decided during the annual meeting.
- Upon reception, the approval committee shall examine and evaluate the application form thoroughly.
- If the application meets the association’s requirements, it will be approved
- If the application does not meet the requirements, the application will be denied and reasons for denial will be communicated to the applicant.
- Once approved, each prospective member will be issued a unique GBA ID. For records and claims purposes only.
- Once approved, the member/s will be on probation for 90 days.
- During the probation period, members will be obligated to pay the required fees of membership.
- Members who are current on their due’s payment shall be classified as “ACTIVE” members in the 91 date.
- ONLY ACTIVE members shall qualify for the Benevolence benefits or claims.
- All applicants will be required to provide proof / documents of the person to be added after the application has been approved, such as marriage certificate or birth certificate for a newborn baby.
